Cellucor "Challenge" Promotion Instructions

1) Purchase a Cellucor product in at least one of the three categories shown below:

• Pre-Workout: M5 Extreme
• Testosterone Booster: P6 Extreme
• Thermogenic/Fat Burner: D4 Thermal Shock

2) Use M5 Extreme, P6 Extreme, or D4 Thermal Shock as directed on the label

3) If after 10 days of using M5 Extreme, P6 Extreme, or D4 Thermal Shock you do not believe that M5 Extreme is the best Pre-Workout (w/ creatine) supplement you have used, P6 Extreme is the best Testosterone Booster you have used, or that D4 Thermal Shock is the best Thermogenic/Fat Burner that you have used, then Cellucor will pay for you to receive a replacement supplement as follows:

• Subject to the terms and conditions beginning at paragraph 4 below, if you used M5 Extreme and were not satisfied that it was the best Pre-Workout (w/ creatine) supplement you have used, Cellucor will send you one Pre-Workout (w/ creatine) supplement of your choice, whatever the maker or brand, so long as the retail value of the supplement you choose does not exceed the cost of M5 Extreme you purchased.
• Subject to the terms and conditions beginning at paragraph four (4) below, if you used P6 Extreme and were not satisfied that it was the best Testosterone Booster have used, Cellucor will send you one (1) Testosterone Booster of your choice, whatever the maker or brand, so long as the retail value of the supplement you choose does not exceed the cost of P6 Extreme you purchased.
• Subject to the terms and conditions beginning at paragraph four (4) below, if you used D4 Thermal Shock and were not satisfied that it was the best Thermogenic/Fat Burning supplement you have used, Cellucor will send you one (1) Thermogenic/Fat Burning supplement of your choice, whatever the maker or brand, so long as the retail value of the supplement you choose does not exceed the cost of D4 Thermal Shock you purchased.

4) To have Cellucor pay for a replacement supplement of your choice, you must fully complete paragraphs 1-3 above, AND you must mail to Cellucor at the address shown below:

• A Hold Harmless Form signed by you. The Hold Harmless Form is available at Cellucor.com/challenge and provides that Cellucor is not liable for any damages that may be caused to you by taking a replacement supplement provided to you by Cellucor under the terms of the “Best in Category Challenge”;
• A completed Replacement Supplement Request Form, available at Cellucor.com/challenge; and
• The Cellucor product purchased by you with a minimum of 50% of its original contents remaining in the original package and an original store receipt showing that you purchased M5 Extreme, P6 Extreme, or D4 Thermal Shock within thirty days of the post mark date on your request for a replacement supplement.
• Mail the Hold Harmless Form, Replacement Supplement Request Form, the remaining contents of the Cellucor product that you purchased along with the original store receipt together to:

5) All requests for a replacement supplement, which must include a Hold Harmless Form, Replacement Supplement Form, the original packaging of the Cellucor product with at least 50% of the contents remaining and the original store receipt, must be received by April 30, 2010.

6) There is a limit of one replacement supplement per person for each category (Pre-Workout w/ creatine, Testosterone Booster, Thermogenic/Fat Burner). The total retail value of replacement supplements per person may not exceed $300.

7) From the time Cellucor receives your Hold Harmless Form, Replacement Supplement Form, and original store receipt, please allow 1 to 2 weeks to receive your replacement supplement.
